"i did not have a visit. My Dr retired, recommended Dr Perez at Westside Medical. I have 2 prescriptions i need refilled, Express Scripts send the forms to Westside, weeks ago. They have not been returned, this morning I made the request by the automated phone to Westside. I am on warfarin and do my INR 2 times a month, Roche has faxed the forms to Westside to be returned to them, this was just after ..."
