"Left me for dead...didn't complete insurance paperwork..tooth rottimh away...sent me home nov 2nd. Without temporary tooth...NEVER COMPLETED PAPERWORK...WENT TO INSURANCE DIRECTLY TO INQUIRE. THEY WERE CAUGHT SCAMING ME...LIARS...I ASKED FOR THE DENIAL AUTHORIZATION REFERENCE # WHEN I WENT TO THEIR OFFICE .... THEY DID NOT FIND OR PRODUCE IT BUT YET SAID I WAS DENIED...HORIZON INSURANCE AGREED THIS ..."
