"The medical records are sloppy and inaccurate. When I tried to have my medical records corrected, the "manager" told me the doctor would not do that and told me I had to have an in person office visit if I even wanted to discuss the matter. Apparently I received smoking cessation education at my last office visit, but I don't even smoke!! I have been a patient for over 10 years, but sadly I will ..."
