"THE OFFICE MADE MULTI MISTAKES ON INSURANCE COVERAGE. APPOINTMENTS. AND MAJOR GIVEN INFORMATION OF PROCEDURE. SAID ONE THING . THEN DR. WANTED MORE MONEY AND CHANGED WHAT HE WANTED TO DO. NOT WHAT THE PATIENT WANTED ADDED ALMOST $2,000 PER PATIENT. OVER THE $2,000 , BILLED PER , TO INSURANCE. TOTAL $4,000 PER PERSON. FOR SIMPLE PROCEDURE."
