"The np made up symptoms in my chart, did not do an Sri screening or an annual pap like I asked. I asked her not to make up symptoms. She was unable to preform the procedure and scheduled me for an ultrasound. I also asked for my annual and and Sti screening. She asked not to do those in my visit. When I came back for the ultrasound, there were symptoms in my chart that I don’t have, I told the tech ..."
