"Avoid this practice! Very poor service. I have been a patient of Dr. B. Khan for more than a year. In April I dropped off a one page employer form related to family leave to be completed and returned to me, for which they charged $50. After eight weeks, I have no form and no refund. I have enquired by phone, but have received no explanation!"
