"Beware if you have insurance! They collect money from you upfront (to have on hand) and then never submit your insurance claims. Or if they do, they are missing a tooth number, or missing a provider name, so that they are denied. It's a vicious circle chasing them. They keep collecting your money, so they have no incentive to submit the claims. They don't want the reduced insurance prices. They ..."
