"Doctor prescribed supplements incorrectly With side effect s w/onconsult or review of Medical history. Balaguerrav Did not prescribe necessary medication indicated by my medical records. My record said surgery recommended instead he recommended flax seed hiding imaging results. South covers for his malpractice. Record phone or office visit appointment, he lies"
