"I had a routine upper endoscopy with Dr. Ellen Dy, wherein someone involved in the procedure; the OR anesthesia prep nurse, or the 'professional' anesthesia administration nurse, Nathan Moos, or ??? broke off my #10 left-front tooth at the gum line.
