"Ooh, I'm really uncomfortable with this place. First, they're unethical. Lied to my face about a "state required" visit to get a form filled out. When I called the state to ask, it's not true. So it's a money grab by this practice (for a learner's permit form). The form can be filled out within 6 months of an annual physical - they told me AT the physical we had to have a separate, state required ..."
