"I have been going to this clinic for 3 years with my whole family . I had my annual cleaning and x-rays in July. Patty, the receptionist, asked if anything had changed on my end regarding address and insurance, to which it had not. Didn't pay anything for that appointment (because that appointment was preventative care and covered 100% by my in-network insurance). But I needed to come back a week ..."
