"I submitted my paperwork electronically, which was a prerequisite before even being allowed to schedule an appointment. Once the office received it, they called and we set the appointment, which was a month out. Without my knowledge, Chris, the office scheduler, cancelled that appointment because I did not confirm over the phone. At no point in the mountain of paperwork was I ever told this was required, ..."
