"Don't surrender your healthcare to this practice if you care about your patient rights. As a previous patient, I requested my medical records and Dr. Mark Sylvester and his staff deliberately withheld important documents and forged a signed release of information document. I had to complain to the Office for Civil Rights at Department of Health."
