"Visited the doctor today for medication refill for blood pressure medication. The doctor put in wrong birthdate in the system. This was not my first visit to see him. This caused a problem with my prescriptions being filled. The office blamed the pharmacy. It was not the pharmacy's fault. I will not be returning to the doctor's office I found a new doctor."

"Had all kinds of problems this morning. On Friday, my doctor wrote a slip for bloodwork, etc. The Phlebotomist noticed that the form was not completed properly and could not proceed unless someone in the front office who could verify the paperwork. I found the lady that worked me on Friday, and she completed the paperwork. I then proceeded to get my bloodwork, etc., completed and found that, somehow, ..."
