"My visit was much too long. I have fibromyalgia which resulted in a flair due to the length of time (1 1/2 hrs). I should have been given the health forms prior to my visit. I suspect corvoid is the reason for the slip-up. I do appreciate the cleaning. My husband has an appointment this coming Tuesday. He has been told how to access the Health forms so things should be better."

I remember as I lost my temp dental crown and stopped by the office without an appointment last year, people from the other group immediately made a new one for me due to my dentist (Dr. Block) not working at that time.
