"I went for an initial visit with Dr. Tien for toenail fungus that had not responded top Econazole Nitrate cream that had been prescribed by my dermatologist a year earlier. Having to undergo a complex spinal surgery followed by the replacement of my right hip and subsequent recoveries prevented me from seeking further treatment until now. Based on my "new patient" appointment with this doctor I would never recommend her to anyone. Upon entering the room (after I'd waited some 15 minutes in the room) she did not introduce herself but asked the reason for my visit. I found this a bit odd since I'd filled out an extensive questionnaire which was emailed to me prior to my appointment and sent back to her office and I was sitting in front of her with my bare feet and unsightly toenails. She had me settle back into the exam chair (which took some effort due to my being stiff as I'm still recovering from my surgeries) and raised it up to look at my feet which she barely looked at. I proceeded to explain being embarrassed about the appearance of my feet. Her response was to almost chuckle as she said well you are in your 70's ... I responded that had just turned 69 which did not put me in my 70's. Here I must digress to say, mask or no mask, I do not look anywhere near my age. I am petite, slim, well-groomed, and I'm often told remarkably youthful in appearance. She just went on as though I'd said nothing, telling me that medically I wouldn't be harmed by not treating the fungus. She couldn't seem to understand why I would be concerned about the appearance of my toenails (at my age --implied). She noted that the cream my dermatologist had not worked, saying that creams have a hard time penetrating nails that are thickened . She noted that thickened nails are difficult to cut but again present no health risk and that some people opt to leave them alone or have their toenails removed altogether. She wanted to know what my goals were, and I told her to have normal nails, wear toenail polish and be able to wear sandals in the summer. She then had the audacity to tell me that I could still wear sandals (right, with my troll feet and what good look that would be). The conversation was unlike any I've ever had with any physician or surgeon, in that she was so flippant in both her manner and words. The upshot was that laser wasn't good for me because insurance doesn't cover it and it can be quite expensive (what's a little more judgement at this point) and the best option, given my goal was a three-month course of oral medication that carried the possibility of liver damage. She was happy to prescribe the oral meds if a blood panel showed me to be a suitable candidate. She did not further discuss risk benefit ratio, whether liver damage if it occurred would be permanent or any other pertinent information. She said she'd write a script for Quest to do my blood work and I told her I preferred the Hoag Lab. She failed to understand why I'd want to go to Newport Beach, and I let her know that Hoag Irvine's lab was in the building next door to the one where her office is. She said she'd write the order and said goodbye leaving me in the still elevated chaise style chair she's neglected to lower to a normal height. The entire unsettling encounter took about 11 minutes from start to finish and to call it 15 minutes would be stretching it .... I know this because my husband accompanied me since I still need help with getting my shoes on. "
