"I found this individual to be rude, inconsiderate and unprofessional. Lacked the courage or maturity to address my questions with accurate, respectful answers. Falsified my medical records and has continued to deny me answers to questions I have presented. Having encountered hundreds of medical professionals in my time as an administrator of a facility for the developmentally disabled I have never experienced such a blatant lack of ethics. "
