"I’m a 71 year old woman with dislocated and degenerative jaw joint discs. I went to see Dr. Cain in June. I have rheumatoid arthritis and it’s very important that I be able to exercise. Immediately after exercising, I’d have pain in my jaws and teeth. Dr. Cain suggested wearing a night guard which I did. He also recommended Botox injections into the muscles right below jaw joints. From that point on, my pain increased significantly. Not only could I not exercise, but I began having trouble chewing many foods. I get tremendous pain in my jaws, teeth, accompanied by headaches. I began physical therapy and went back to see Dr. Cain. On July 11, he recommended Open Bilateral TMJ Arthoplasty, and said I’d be hearing from the surgical scheduler. I did hear from her about 5 days later and she told me that Medicare wouldn’t cover the surgery and that Dr. Cain’s fee was $24,000, which didn’t include the surgical facility or anesthesia. She gave me a procedure code of 21240, and I called ...
Medicare myself and was told that it was covered and that the Dr. needed to call the “provider line” and get prior authorization. The person at Medicare said documentation may be requested. I had my MRI results, a letter from my rheumatologist stating the need for the surgery, and could get documentation from my physical therapist stating that I’d been getting treatment. Dr. Cain’s surgical scheduler said that the procedure code was never covered by Medicare. I have a Medicare supplemental policy also, but it doesn’t pay if Medicare doesn’t. Numerous emails went back and forth stretching out over about a 6 week period. I talked to Dr. Cain twice and he told me he had done surgery on Medicare patients multiple times and couldn’t understand what the problem was. He also said he’d be happy to call whom ever and asked if I had the provider phone number. Medicare won’t give that to the patient for obvious reasons. I again talked to Medicare and was assured that the procedure was covered. I also spoke with the billing manager over all 13 locations of Austin Oral & Maxillofacial Surgery group and she stated that Medicare would not cover the procedure. I was told by the surgical scheduler paperwork could be filed after the surgery and there may be some payment. I cannot possibly proceed without prior authorization. After being told conflicting information between the doctor and his staff, I “finally” was told that they are a non-participating provider of Medicare. I should have been told this from the very beginning. So much time and energy has been wasted all this time. I don’t know of anyone requiring this surgery that would be willing to undergo it without prior authorization. You could easily have charges in the $50,000 range overall. I believe Dr. Cain to be an excellent surgeon, but there is a definite disconnect in this whole process. I am left in pain, frustrated, and very disappointed. "
